Insulin Syringe Units to mL: Reading the Barrel
On a U-100 insulin syringe — the standard almost everywhere outside veterinary use — one unit equals 0.01 mL, and the full 100-unit barrel equals exactly 1 mL. The "units" printed on the barrel are not a measure of substance. They are a volume scale, and converting them to millilitres is a matter of dividing by 100.
The word "unit" is doing something unusual here, and it is the source of most of the confusion. In almost every other context a unit measures an amount of something. On an insulin syringe it measures volume, and nothing else. A unit marking tells you how much liquid sits in the barrel — it says nothing whatsoever about what is dissolved in that liquid or how much of it there is.
The conversion
A U-100 syringe is calibrated so that 100 units occupy 1 millilitre. Every unit is therefore one hundredth of a millilitre.

- Units to mL — divide by 100. 40 units ÷ 100 = 0.4 mL.
- mL to units — multiply by 100. 0.15 mL × 100 = 15 units.
Barrel sizes and what changes between them
Insulin syringes are sold in three common barrel capacities. All three use the same U-100 scale, so a unit means the same volume on each. What changes is the total capacity and — more importantly — how finely the barrel is graduated.

The graduation interval is the detail most people overlook. A 1 mL barrel is typically marked every 2 units, which means any odd-numbered value has to be estimated between two lines. A 0.3 mL barrel marked every single unit gives roughly three times the readable resolution over the range it covers. When precision matters, the smallest barrel that covers the volume is the one that reads most accurately.
The U-40 mix-up
Not every syringe marked in units uses the same scale. U-40 syringes exist — they are standard in veterinary settings in some regions — and they are calibrated so that 40 units occupy 1 mL. On a U-40 barrel, one unit is 0.025 mL rather than 0.01 mL.

Reading a U-40 barrel as though it were U-100 produces a 2.5× volume error in either direction, with no visual cue that anything is wrong — the numbers on the barrel look ordinary. The scale is printed on the syringe packaging and usually on the barrel itself. It is worth confirming rather than assuming, particularly with syringes bought outside the usual supply chain.
Gauge is a separate measurement
Needle gauge describes the outer diameter of the needle, and it is unrelated to the volume scale on the barrel. Counter-intuitively, the gauge number runs backwards: a higher number means a thinner needle. A 31G needle is finer than a 27G.
Gauge and length are typically printed together — 31G × 8 mm, for example — and neither figure has any bearing on how the barrel graduations should be read.
Why units alone can't tell you an amount
Because units measure volume, the same unit marking corresponds to a different quantity of material at every concentration. Volume is only half the calculation; concentration is the other half, and it is set by how much diluent was added to the vial.

No — the scale is inverted. Higher gauge means thinner.
